  2nd 2 boxes: email and confirm email
  3rd 2 boxes: street address and city
  4th 2 boxes: state and zip code
  next box, I just put fb
  Check off that little box below, and click “SOMETER”.

  It said I already requested one also. I guess if you have already gotten the English version of this sample, it won’t let you get another. That is the only other sample of this that I have requested, so that’s what I’m guessing.

    Its not a new one. If you already requested (from the English language offer a few weeks ago) they have that on record and will send you a new one.
