Log into your Amex account. It should be listed under “Your Offers”. You have to click on a button to accept offer. You must use that card for purchase & Amex will credit your account $5. I only recently found out these offers!

Question — the rebate states I need to give the packing slip & UPC code but it was shipped to my local store and picked up so I don’t have a packing slip. Anyone know if the receipt will work instead or how to get a packing slip?
Note folks, the rebate only applies for one item. If you purchased more than one hoping to get a rebate on all of them (like me), you will be disappointed. You will only receive a rebate for ONE, any others ordered under the same name/address will be denied. Unfortunately, the rebate form said “one name, per envelope, per address” so I thought I could get rebate for each as long as the forms were submitted separately…My advise, have someone else purchase extras for you.
