    Thanks Collin!!! :] The website seems to be running a little slow, as there was definitely a delay once I clicked submit. And I haven’t yet received a confirmation email either. (Probably slow.) But, I was able to submit the form/free sample for both myself and my mom.

  You have to change the info on form… I changed everything but the address and got two. Must be 1 per person (even though I didn’t read that anywhere) and 2 per household.

  I was able to snag two … One for my mom and one for myself. I always read the Terms & Conditions, as we live at the same address and occasionally, albeit rare, there are some freebies that offer more than one per person/household. It’s always wise to read the “fine print.”
