The connection keeps timing out for me. Hope the rest of you are able to get through!
it takes a while to load. i was able to fill in my info and then when i tried to submit, i got a connection timed out message.

The link on the Costco site takes me to a page that isn’t available. Anyone else having this issue?

It worked OK for me. When I clicked on “Get Sample” button, I got the connection message. I refreshed the page and was able to fill out the form. After I submitted the form, it took a little while to get the confirmation page, but it finally loaded. It says I will receive my sample in 3-5 weeks.

Loaded pretty quick with firefox. But I filled it out wrong, answered female for sex thinking they were talking about me, duh…my poor little boy is going to get princess pants!
Gender was in reference to you. You had to pick boys or girls & the size in a dropdown box.
