   It gave a general “oops” error message and said to hit the button to return to the form and re-submit. I finally figured out that sample request would not go through with the response selected: “I have never heard of Truvia before”.

   It went through fine for me when I selected that I use the product. Thanks for posting as I do use this daily in my tea.
