Every time I click submit it says “Failed to submit form.”
same thing keeps happening to me
If you don’t check mark for them to contact you by email it will not submit and you have to reset the form and refill it out .. same thing happened to me and I finally figured out what I was doing wrong

I see it (looks like you need flash), but am getting the failed to submit form message.
It told me thank you the first time I submitted the form, and it just reset the form. It wasn’t obvious that my submission went through, so I submitted again, and get the Failed to submit form…..
BTW… Collin, I love the winking emoticon…. LOL!!
when i first went to the site the free sample form didn’t show up. So I tried going to the site again and it was the first thing that came up on the screen. Then when I submitted it wouldn’t submit. I unchecked the box and clicked submit and it worked…you will see a thank you and the form will be blank, that’s how you know it worked.

I thought I needed to resubmit this form, but when I tried, it looks like it had gone through. But it also said, “You can request only one sample per day.” Can we do this daily? I’ll try again tomorrow!

The sample form also said Failed to Submit for me as well. I just clicked submit again (twice) and it finally said “Thank You”… So I’m thinkin’ it went through.

I just kept hitting submit and then on like the 10th time it said thank you!
This is what happened to me too. I reset and refilled out the form to have the box checked and then just clicked the button over and over until it went through.
Same thing kept happening to me! I just kept clicking “submit” and it finally said “thank you!” Hopefully I got it!

I had to hit resubmit after checking their email box and then it went through.
I hope my mailman doesn;t mind all the little boxes i am going to be getting free sample wise!

Worked fine the first time for me.
After I summitted the first time, with box checked, it showed the form blank, seemed like a error, so I had nothing on my screen to click submitt again, and so I filled it out again and re submitted it to get the one submission per day like other of you, I am sooo confused if it went through, the screen is not easy friendly to read so I am not sure if I ever got a thank-you or not.

The first time I did it..it just went back to the blank form where I had started..so after reading the comments here..I went back to try again (and click the box which I didn’t do last time) And it said I can only submit one sample per day..so I’m guessing it went through the first time..yay! I hope so 😉
It kept tell me I wasn’t 18 years old when I put in 1984 than I tried a bunch of different years (even 1911) and it still told me i wasn’t old enough???
