1) You can purchase other merchandise with it, but you only get one receipt and if you purchase 2, you’ll need to make sure they are on different receipts.
2) the form looks like it only has room for one UPC to be attached, so you may need to submit each one by itself.
3) The form does say you original store receipt must be included and it can be online or instore.
