Marie Clare digital is the default, perhaps you didn’t click on it? I clicked on Food Network Paper and it worked for me.

The forms seem to be messed up and will not let me type any information like name and address. Their contact us form is the same way.

You can also chose to take $10 instead of a magazine subscription. To redeem for the cash reward instead, go to https://www.bperx.com/ and click the link to go to the forms page.
You have to print the page, fill it out and mail it. You just have to fill out your name, address, email and phone number plus the gift voucher number.
