  Click the tab Collin got it as above. Then you wait a little bit until you can see small yellow tab under the bottle. The tab should be take you to application. Hope it’s works.

   I believe this asks for a membership number. Not sure if it asked that before as well?

  Collin stated above that “you do NOT need to be a Sam’s club member, yadda, yadda…..” thus you would not have to put a membership # in.
