I believe Tammy was referring to yourhealthyeyes.com. It comes with a big ad then prints at the bottom like the one Chris mentions below.

I go down to the bottom right and click the Help button and a form should pop up to fill in but if it doesn’t and a lady pops up look down to the bottom of that page and you’ll see Mail My Coupon and Click Here to the right and then you can fill in your info”!
