My form doesn’t have words on the side to indicate name, email addresses, etc. How did you fill it out?
I just guessed! Mine was the same way
When I clicked on autofill it did, first name, last name, email, street address, city, state and zip
First name
Last name
e-mail
Address
city, State, Zip
Left box on bottom to state over age of 18

Never got a success message but when I tried to submit another time it tells me I can only enter once…I hope that means it worked!
there is something wrong with the system right now.
I had a partially loaded page in which I was able to fill out the form. The image links were broken and all though. I had to click “submit” a few times, then I refreshed it after and an image popped up saying along the lines, “Now that you got one, share this with your friends!” The page was still half broken though. We’ll see. Thanks, Collin!

I think/hope I got one–would really like to try this. I finally got a form to fill out, after refreshing several times, hit submit and now it’s a blank page again…
After many attempts, it finally told me enjoy your yogurt now share the joy with your friends by hitting the share button above.. Your coupon will be mailed within the next 3 to 4 weeks.
I think everyone wants free yogurt so the page is taking a looong time to load.
ok everyone try again. I actually called into them. Told them their facebook page was down. They are willing to mail you a coupon booklet if this does not work for you. You might not get that item for free but you will get extra coupons. So I tried this on my hubbys page went through like a charm. My page said sucess. When I tried it again I got only submit once. So we will def be getting one coupon but maybe two. Can I just say gyro! Yum can’t wait to get this!
Signed up for my daughter and it went through, saying coupon being mailed in 3-4 wks.
Did mine again and it said can only enter once, so it seem to have gone through, even though mine said Success before.
Got one so excited to see plain yogurt.
