I was able to fill out the form. It said something like my information had been received. But this also looks like something you can purchase for $1 anyways?
It is a catalog of the products they have in store, and online. I received last time. I decided not to request one this time.

It will say promotion over if you leave the apartment # field blank. When I put “drive” in that field it went right through (the last of my street name). Hope this helps.

Thanks for the tip on filling in all the boxes. The first time I didn’t put anything in the apartment box, saw the comments, went back and put “none” in the apartment box. It said it received my information, so fingers crossed.
Put all info in, afterwards went to another screen, tried logging in to see if it worked, nothing happened, tried signing up again and says I already signed up, so I will see what happens.
