I had that same issue when using Google Chrome, had to use IE (FF would probably work too).
I had to click on reset form in that section for the gym info and then it worked.
will see I really wanted to get a ball I did order it but is this a small ball or the big ones because I’m 5’4″ and always have to grab the smaller balls at the gym?

You don’t need a membership number. It says to skip this step if you don’t have one. Although I didn’t show anything in that field it was not letting me go on. I hit reset in the membership section and then hit submit and it all worked without any problems. Hope that helps. 🙂
