Here’s what a customer service rep from Kohls told me regarding rebate submission.
Thank you for contacting our rebate processing center. I am sorry for any confusion this process may have caused you. If both products are eligible for the same offer, only one rebate form must be submitted including the original packing list and the original UPC barcodes from each product purchased.
Once the rebate has been approved, then one card with the total amount you should be entitled to received will be delivered.
So it seems that we can send in multiple qualifying UPC in one submission.
can we submit for rebate online?
I got the Black & Decker can opener they had with the rebate but my UPC # does not match the UPC # on the rebate form. My invoice has the correct # but the can opener does not.

Katie I contacted Kohl’s through facebook and was told that the company was aware of the misprint and to send it in anyway. I sent it out so we’ll see.

Thank you SO MUCH for having these rebate form links on your page!! I went to Kohls.com and they are NO WHERE to be found. I usually have great success with Kohls, and I’m really disappointed this time. I think I’ll go the Facebook route to communicate, because calling the Kohls.com help line was NOT helpful.

I need zip code for rebate address. El Paso, TX, Dept 13-77999, P. O. Box 540009. AND is the Dept # correct? #13-77999 ?? Thank you. I’ve asked the store in Joplin, MO and called the phone on the rebate form. No one could tell me.
