I submitted the form last year for the 5 coupons and had no problem getting or redeeming them
I submitted my $3.00 rebate coupon on 11/17/15 and it says expect 8 weeks for payment. Have not received anything. Is there a phone number for the Scottsdale, AZ location that I could check with them. Also, submitted the coupons for 2016 and haven’t received any coupons yet. Would like to know when to expect these.
