Hmm I got an error the first time I submitted the form, and when I filled it out a second time and hit submit it reset my form with no confirmation. I guess I’ll just have to wait and see if anything shows up.

I give up. Thank you anyway. I tried y times. Got an error message the first time, clicked submit the second time and it just kept spinning, 3rd time there was no submit button, 4th, 5th and 6th time error message.

Ok so I got it worked out! I am on my iPhone, so I opened it in Safari. Filled out the form, clicked submit. It disappeared, but I waited on that page for less than a minute and the confirmation popped up! So keep trying and be patient. 😊

I just retried and it said only one per household so I’m assuming it accepted my info earlier despite the error page coming up. Seems to be working now.

Yay I got one too! It gave me the error screen, then I hit the back button and while I was trying to decide whether to hit submit again or not, the page re-loaded itself to a different form with the same questions. I had to fill it out again, but then it said it went through after I re-submitted!
