The fine print says to get both rebates it must be submitted by two different names and two different addresses. Could I use my husband’s name and our home address with my name and work address?

I bought this at Costco last year and I am not sure if I am the only one who is not thrilled? The darn thing reads different temperature when you use it on same person within a minute. I find it difficult to use and unreliable? Am I doing something wrong – not sure, I followed instructions to the T. As a verification I tested temperature on two other thermometers at a same time , one digital and one manual and while those two had same temperature the Exergen had completely different one.
No, you are not the only one, I have the same problems you’re having.
The rebate forms themselves say: Limit one rebate per item. I am not sure that both rebates can be used. Can anyone confirm?
The only difference in the rebate offers is that one is going Rebate Offer/BT‐FF and the other to Rebate Offer/BT‐M. When I worked in Corporate America, I handled all the rebates for our particular product line. If 2 rebate forms were received from one person/addy, but sent in to different “Box addresses” we were to return the second rebate received, to sender.
Here’s how this looks to me – you send the UPC and the receipt with one of each rebate forms. It looks like it’s designed to be purchased for a baby shower. So one of the $10 rebates is filled out for the baby shower recipient, and the other is filled out by the person who purchased it for them – friend/family. So you’d need 2 different addresses, and 2 different names, to pull this off. The limit is one rebate per ITEM – which to me means 1 mother’s rebate, one F/F rebate, but 3 rebates per person, so you could theoretically do this 3 times. You need an accomplice to pull this off, but it’s doable.

So mine arrived from Costco.com and there is no receipt; just package labels from UPS. Is that what I mail in to get the one rebate.. There was nothing like an original sales receipt which is what they ask for. Does anyone have an idea what to do?

I am not sure how to send the rebates either. MIne just arrived and after reading peoples comments, it sounds like this is tricky to get the rebate.

One requires the receipt and the other requires the barcode. You may want to send them separately to avoid any confusion. Can you send in both rebates together? The coupon says “Limit one rebate per item. Maximum 3 rebates per person.” I also have a $5 manufacturer rebate (making 3 rebates total for the 1 item) but wondering if I need 3 receipts to get the 3 rebates?
