I read over that too, but after submitting my first request. Surprised there’s nothing on the form about those fields being required. I went back and requested a second one and made sure to check each box. Haven’t gotten another confirmation though.
I only got email conformations for my hotmail accounts and not yahoo account.

It let me request more than one! After I did it, it told me I had already requested my three the next time I tried. (<<I hope that makes sense…couldn't figure out how to word it! LOL)
However, I only got ONE email confirmation, so we will just have to wait and see if they actually come…

The site is SLOW…I had to refresh it over and over again before I finally got through. I did NOT opt to receive email communications from the sponsor and I still received an email confirmation saying I would get the onesie, catalog, and 20% off coupon in the mail!

Funny that no one has mntioned that the field for STATE is missing on the form. Haha. There are two fields for ZIP CODE.
State isn’t missing, that’d be a huge problem. The H&M site is having troubles loading for many people, yours must’ve loaded incorrectly.
